Student Medical Records
Hello New and Returning Families,
We are looking forward to seeing your children this fall! To prepare, every student must submit an annual record of physical exam and immunization certificate. Please submit your medical records form by August 12.

Immunization Certificate (required for all students)
Medication Administration Form (required for all medications to be administered on campus) — NOTE: All medication must be in its original packaging and the information on the pharmacy label must match all of the information on the uploaded form from your doctor, including:
Name of student and birthdate
Name of medication, dosage, route
Name of physician, address, telephone, and
the medication must not be expired.
Food Allergy & Anaphylaxis Emergency Care Plan (“Allergy Plan”): If you have an allergy plan, in addition to submitting a “Medication Administration Form” for each medication noted in the plan, please work with your doctor to complete an Allergy Plan.
Asthma Plan: If you have an asthma plan, in addition to submitting a “Medication Administration Form” for each medication noted in the plan, please work with your doctor to complete an Allergy Plan.
Allergy / Asthma plans / doctor’s orders are considered up to date if they are due to expire after the last day of school: June 17, 2025.
Family Meeting with Nurse Nancy Hoover
If you are submitting the Medication Administration Form, an Allergy Plan, and/or an Asthma Plan, you must schedule a household meeting (all grownups and student(s)) with Nurse Nancy Hoover.
